To the State Superintendent

Madison, Wis.

 We, the undersigned member of the school board of Joint  district No. 1  of the town (or towns, village or city) of Unity + Brighton + Vil Unity counties of Clark & Marathon, state of Wisconsin, hereby apply for special state aid for the above named district on account of transportation provided persons of school age (4 years and less than 20) residing in said district for the year ending June 30, 1932.

A. Consolidated Rural School District

1. Was transportation provided to and from the consolidated school for the entire school term for all children residing over one mile from the school house?………..

        NOTE---Report names, etc., Table 1.

B. Districts (not consolidated) in which the school has not been suspended.

1. Did the electors at the annual meeting, or a special meeting legally called for that purpose, authorize the school board to provide transportation for children residing in the home district? Yes

2.  If so, for how many months was transportation provided? Nine

3. If the electors failed or refused to authorize the school board to provide transportation, was transportation which was safe and comfortable provided by the parents or guardians of children residing more than two miles from the nearest school in the home district or adjoining district?________

Note.---If so, report such children under Table II; give all information called for.

C. Districts in which the electors have voted to suspend all of the schools and new districts formed out of detached territory with no school building.

2. Did the electors authorize the payment of tuition and transportation for such time as the school was suspended for all persons of school age, desiring to attend public school in an adjoining district or districts?……..

D. Union Free High Schools.

1. Was the transportation provided by parent or guardian for children residing more than three miles from the high school in such district, safe, comfortable and convenient?__

E.  The questions below must be answered by all districts except those in which electors fail to authorize the board to provide transportation, and transportation was provided by parents or guardians.

1. Did the school board enter into written contracts in the name of the district with the parents or guardians or other persons for transporting or providing for the transportation of all persons who attended and who were entitled to transportation? Yes

2. Did Such contracts provide that the children were to be transported in a safe and comfortable manner in a conveyance provided with protection against cold and inclement weather? Yes

3. Was the driver of each conveyance a person of good moral character, trustworthy and responsible?_____

4. Did such person or persons, other than parent or guardian, who entered into contract with the district file a bond in the sum of $250,000, as required by law? Yes

5. In case board and lodging were provided in lieu of transportation, did the school board enter into written contract with the person or persons who furnished such board and lodging?_______

We, the undersigned school board members of the above named district, each for himself and as an officer of said district being duly sworn, depose and say that the forgoing statements are, according to our best knowledge and belief, true and correct in every particular and in each and every case the aid is demanded for the number of days such child actually attended school while being transported to and from school in a safe and comfortable manner or boarded as provided by law.  Further, that no claim is made for any day or part of day when such child was suffered or permitted to walk to or from school.
