Application for Special State Aid for Transportation, Section 40.34 For the School Year ending June 30, 193__ Note: Read carefully all directions on page 4 Transcribed by Ken Wood To the State Superintendent Madison, Wis. We, the undersigned member of the school board of Joint district No. 1 of the town (or towns, village or city) of Unity + Brighton + Vil Unity counties of Clark & Marathon, state of Wisconsin, hereby apply for special state aid for the above named district on account of transportation provided persons of school age (4 years and less than 20) residing in said district for the year ending June 30, 1932. A. Consolidated Rural School District
NOTE---Report names, etc., Table 1. B. Districts (not consolidated) in which the school has not been suspended.
C. Districts in which the electors have voted to suspend all of the schools and new districts formed out of detached territory with no school building.
D. Union Free High Schools.
E. The questions below must be answered by all districts except those in which electors fail to authorize the board to provide transportation, and transportation was provided by parents or guardians.

We, the undersigned school board members of the above named district, each for himself and as an officer of said district being duly sworn, depose and say that the forgoing statements are, according to our best knowledge and belief, true and correct in every particular and in each and every case the aid is demanded for the number of days such child actually attended school while being transported to and from school in a safe and comfortable manner or boarded as provided by law. Further, that no claim is made for any day or part of day when such child was suffered or permitted to walk to or from school.
